PaRK Mental Health Service is offering an exciting new opportunity for a highly skilled and motivated Clinical Nurse Specialist, Mental Health to lead the Wellness Clinic.

The 1.0 FTE Position Will Work

  • 0.5 fte based at the Rockingham Kwinana Adult Mental Health Clinic and
  • 0.5 fte based at the Peel Mental Health Clinic.

The successful applicant will lead the new PaRK MHS Wellness Clinic. The Wellness Clinic will focus on improving the physical health outcomes of clients living with enduring mental health disorders. Clinicians from the Clinical Treatment Team (CTT) case manage suitably identified clients with the principal goals of providing monitoring and the management of side effects and adverse effects of psychotropic medications, Metabolic monitoring, wellness promotion, and where appropriate the supported transition to shared care and primary care arrangements. The successful applicant will be responsible for the day-to-day functioning of Wellness clinic across PaRK. Responsibilities include case management of clients treated with Clozapine, supervision of Clinical Nurses associated with the Wellness clinic, the development, implementation and evaluation of relevant policy and procedure and the collecting and reporting on statistical data related to the Wellness clinic.

The Successful Applicant Will

  • Be passionate about improving the physical health of people with serious mental illness and have advanced knowledge of standards of care in relation to the physical care of Mental health consumers.
  • Have advanced knowledge in the care and management of clients treated with Clozapine and other psychotropic medications.
  • Have a strong focus on working collaboratively with the GP Liaison CNS position and other key stakeholders to transition suitable clients to shared care or primary care arrangements.
  • Will act as a resource for the wider Mental health team and deliver education and training as required.
  • Possess a high level of understanding into the importance of evidencing 'clinical activity' related to the wellness clinic in the Activity Based Funding (ABF) environment.
