A Health Improvement Practitioner is a rewarding, generalist role embedded in the primary care team. You will deliver brief, evidence-based behavioural and psychological health interventions and work collaboratively with GPs, nurses, health coaches and community agencies.

Team Medical is a locally owned practice working in association with Green Cross Health. We are an integrated family health centre passionately providing a full range of GP Care to our enrolled patients and Urgent Care for accidents and urgent medical needs to our community. As a key health hub to the Kapiti Coast our GP services typically operate Monday to Friday between 8am and 6pm. Our Urgent Care services operate 7 days a week from 8am - 10pm. We also offer full X-Ray facilities on site.

About the Position and You

The key purpose of the role is to increase access to wellbeing and behavioural health intervention services in general practice. It involves being an integral part of the general practice team, working directly with individuals and their families to help improve mental health and overall wellbeing. You will be situated in the GP clinic, and help patients with any issue affecting their wellbeing (mood, sleep, relationships, anxiety, grief, stress, dealing with a new diagnosis etc). The Health Improvement Practitioner (HIP) also supports development of general practice teams in providing consultations and advice to its members.

Do you have what we are looking for?

  • Registered health professional
  • Current Annual Practicing Certificate with regulatory authorities within HPCA, DAPAANZ or the Social Work Registration Authority
  • Have some training in evidence-based talking therapies such as ACT, FACT or CBT
  • Clinical experience in mental health related setting
  • Confident, friendly, proactive and approachable.
  • Excellent communication and interpersonal skills.
